The primary purpose of the STS Support Coordinator is to support the STS Team Leaders with administrative tasks around assisting children and families with accessing behavioral, medical, educational, and social services and supports in a manner consistent with values and principles outlined in the DBHIDS Framework.
1. Conducts and/or assists with intakes for children as needed for the STS program.
2. Ensures all required annual documents are reviewed and signed by the consumer’s parent/guardian.
3. Update Case Management notes as needed.
4. Responsible for medical records charts being in order.
5. Complete the CBH Tracker on a bi-weekly basis and upload to SharePoint.
6. Complete school census on a weekly basis and upload to CBH in a timely manner.
7. Responsible for scoring all structured tools and attaching to the psychologist calendar.
8. Assists with the scheduling and preparation of evaluations.
9. Participate/facilitate in psychological evaluations as needed/requested by the STS Team Leader/Director.
10. Manage/update authorizations utilizing the COCA process/format.
11. Assist with electronic billing and processes as needed (ex: enrolling new clients, discharging clients, etc.).
12. Will coordinate with the BHRS/STS Scheduler, BHRS Clinical Supervisor/Manager and STS Team Leaders to enhance and promote collaboration.
13. Communicates and coordinates directly with STS program clinicians regarding evaluation documents, services and other case management items.
14. Assists in administering of the Consumer Satisfaction Survey and other activities aimed at receiving feedback from consumers, families, and school staff.
15. Makes home visits as needed to enhance collaboration with families.
16. Process STS packets to CBH for authorization and discharge packets.
17. Provide coverage for other STS Support Coordinator, BHRS Coordinator and/or BHRS/STS Scheduler as needed.
18. Travel between schools and main office as needed (personal vehicle required).
19. Must demonstrated competency with Electronic Medical Records and the ability to navigate and use computer software programs and e-mail in a competent manner.
20. Complete trainings as required.
21. Adheres to safe work practices.
22. Other job duties as assigned.
23. Must function in full compliance with all applicable policies and procedures, federal state and local laws and Interact’s Code of Conduct. Must participate in Interact’s Compliance Program including reporting suspected violations of the compliance program or applicable laws or regulations to Supervisor/Manager/Director or the Compliance Officer.
1. Bachelor’s degree preferred in social work or related field, and a minimum of one (1) year of full time paid direct care experience or supervised practice in lieu of experience requirement.
2. Two (2) years of care coordination, case management experience and 60 credit hours/associates degree from an approved educational program.
3. Possession of a valid driver’s license from state of residence, a minimum of one (1) year driving experience and a driving record free of at-fault accidents and/or moving violations within the past year preferred. Must possess or have access to an automobile.
4. Valid clearances in accordance with PA Acts 33, 73 & 80.
5. Participation in and completion of all CBH mandatory in-service trainings (must be completed within three (3) months of hire and annually from the date of initial training.)
8. Participation in at least ten (10) hours of documented additional in-service training per year (year is defined as twelve (12) months from date of hire.)
