Community Homes Services Licensed Practical Nurse (LPN)

Position Type: FTE (37.5 hours per week)

Starting Pay Range: $32.00 per hour

PURPOSE:

Under the supervision of the CHS/IDD Director, the nurse supervises medication administration, reviews medical records/appointments, writes and maintains medical histories, follows up on Dr.’s orders, diagnosis and treatment plans as needed, Support Plans as needed, coordinates overall medical participation of each resident including, but not limited to, primary physician, neurology, physical therapy, psychiatric and all other disciplines. 

PURPOSE:

Under the supervision of the Community Home Services, Intellectual Disabilities Division Director, the nurse supervises medication administration, reviews medical records/appointments, writes and maintains medical histories, follows up on Doctor’s orders, diagnosis and treatment plans as needed, Support Plans as needed, coordinates overall medical participation of each resident including, but not limited to, primary physician, neurology, physical therapy, psychiatric and all other disciplines.

FUNCTIONS:

1. The nurse is responsible for the overall planning and coordination of the physical health of the CHS(Community Home Services) residents, including:

2. Visiting each resident on the caseload in their home at least 1 time per month, and completing a nursing note, documenting the visit. In addition, completing and documenting a medication check for each person visited.

3. Reviewing all medical consults and physicals. Follow up with doctor if necessary. Upon review, emailing detailed summaries of documentation received, indicating follow up needed and ensuring that all follow up medical appointments are made.

4. Updating Lifetime Medical Histories annually and as needed when a major medical change occurs. LMH must be updated within one week of any major medical change, including diets, mobility, adaptive equipment, etc.

5. Developing and participating in on-going training for staff on medical issues. Immediate training is needed for any major medical change, including diets, mobility, adaptive equipment, seizure rescue medication etc.

6. Attending all ISP(Individual Support Plan), QEST (Quality Enhancement Support Team), and SIS (Supports Intensity Scale) meetings.

7. Acting as liaison to hospitals, nursing homes, etc., to ensure proper medical follow-through for people using those facilities. Follow up with Physical/Occupational Therapy/Visiting nurses if a resident is receiving these services.

8. Visiting hospitalized individuals within 24 hours of admittance or no later than Monday noon if the admittance occurs on the weekend, speaking with hospital for updates, and participating in discharge planning. Completing a home visit within 24 hours of discharge.

9. Taking verbal orders from doctors, ensuring new prescriptions are filled and delivered to the home and transcribed on the MAR(Medicine Administration Record). If you are sending an email and do not receive a response, please follow up with a phone call to the Program Manager or go to the home and ensure the medication arrived. Also, please make sure it was transcribed.

10. Communicating with the pharmacy as the point of contact for all pharmacy related issues, and going to the home to update MAR (Medical Administration Record) if a verbal order is received.

11. Completing the HRST(Health Risk Screen Tool) Clinical Review for everyone on caseload.

12. Participating in scheduled bi-weekly supervision with the Intellectual Developmental Disabilities Director or Assistant Director.

13. Maintain the agency-issued cell phone and respond to calls within 30 minutes. The on-call supervisor will contact the on-call nurse with questions related to the emergent care of the client and confirm next steps.

14. Participate in a rotating on-call schedule with other nursing staff.

15. Maintaining open line of communication with supervisor, program managers, family members, and all team, members.

16. When the Nursing Assistant is unavailable, appointments will not be delayed if they needed to be scheduled within a week or month.. Nursing staff will assume assigned responsibilities for scheduling appointments and other essential duties to ensure uninterrupted care.

17. Completing IDD(Intellectual Developmental Disability) Annual Training Plan as required per quarter before the due date.

18. Adhering to safe practices.

19. Completing other duties as assigned.

20. Functioning in full compliance with all applicable policies and procedures, federal, state, and local laws Interact’ s Code of Conduct. Must participate in reporting suspected violations of the Compliance Program or applicable laws or regulations to supervisors/managers/Directors or to the Compliance Officer.

QUALIFICATIONS:

  1. Pennsylvania Licensed Practical Nurse or Registered Nurse Certificate.
  2. Two (2) years’ nursing experience, preferably in a residential home environment.
  3. An examination statement which indicates that the employee is free from communicable disease at time of employment and every two (2) years thereafter, which includes TB testing (PPD and/or chest X‑ray).
  4. Clearance in accordance with PA Act 33 & 80.
  5. Must have a valid Childline, PA Criminal History, FBI Clearances prior to seeing clients.
