Give us a call beginning November 1, 2016 to start the application process.

Current Utility Bills/Shut–off Notice
Social Security cards for all household members
30 days of most recent income for all household members
Birth dates for all household members

3 person $2,520.00 $30,240.
4 person $3,037.00 $36,450.
5 person $3,555.00 $42,660.
