Starting Pay Range: $35,000
PURPOSE:
The role of the Recovery Specialist (RS) in the Supported Independent Living (SIL) program is to support SIL residents with their behavioral health recovery while also empowering them to achieve skills to promote independence. The SIL program is a short-term residential program.
FUNCTIONS:
- Empower individuals to actively work on goals so they can successfully transition to living independently in permanent housing.
- Collaborate with individuals, natural supports, care providers, and treatment team members in identifying and developing targeted Recovery Plans.
- Conduct ongoing assessments and support individuals with identifying strengths and needs. This may include risk assessments and monitoring whether a client may require crisis intervention or a higher level of care.
- Provide crisis intervention and support following established crisis safety plans and procedures. This may involve accompanying the individual to the CRC or hospital.
- Promote personal daily living skills (e.g., developing a system for remembering to take medication as prescribed, navigating public transportation to attend medical appointments, budgeting and paying bills on time, housekeeping, etc.)
- Assist with system navigation as defined in their recovery plans (e.g., resume writing or other activities to support educational or vocational goals, accompanying individuals to appointments at Public Assistance and Social Security offices, connecting with leisure activities or other supports including religious communities).
- Coordinate care between all service providers and systems serving the individual. These include but are not limited to psychiatric, medical, dental, educational, evaluations, employment, volunteer, and/or as assigned by Program Director.
- Provide linkage to identified resources with the individual’s consent. This may include facilitating referrals for expanded mental health programming.
- Transport clients to appointments in Interact vehicle.
- Complete and maintain all required records and documentation in compliance with agency, county, and state standards.
- Participate in unit activities and events that promote community engagement.
- Coordinate property management and maintenance with the agency’s operations department and external service providers.
- Track/update individuals’ progress weekly.
- Provide weekly unit inspections for cleanliness and maintenance needs.
- Monitor SIL resident compliance with SIL Handbook.
- Assist individuals who are ready for permanent housing with identifying new housing.
- Ensure that rent is paid on time by the individual.
- Provide on-call services.
- Actively participate in supervision, program, and agency meetings and training as delineated in an annual training plan or as assigned by the Program Director.
- Function in compliance with agency policies/procedures and federal, state, and local laws.
- Participate in Continuous Quality Improvement (CQI) activities as requested by Program Director or Compliance Officer.
- Performs other job-related duties as assigned.
QUALIFICATIONS:
- RS’s must meet one of the following criteria:
- A Bachelor’s Degree with academic concentration in social services with 1 year experience preferred.
- An Associate’s Degree with academic concentration in social services with 3 years experience preferred.
- A high school diploma or GED with 5 years experience in social services.
- Ability to work independently, as part of a team, and share office space.
- SIL is a residential program and requires the ability to work a flexible schedule including a combination of day, evening, weekend, and some holiday hours.
- Must be able to lift at least 50 pounds.
- Must have valid Childline, PA Criminal History, FBI Clearances before seeing clients.
- Possession of a valid driver’s license from the state of residence with a minimum of one (1) year of driving experience and a driving record free of at-fault accidents or moving violations for the past year.
SKILLS:
- Positive interpersonal and communication skills.
- Flexibility, creativity, and ability to resolve challenges innovatively.
- Proficient computer skills. Experience using electronic medical records preferred.
